Mine is through Fidelity, and is very easy to check via the app. I check 1-2x/week.

I check all of my accounts once per month. And at the end of the year I sometimes rebalance some of them or change the % that's being put into each of the accounts.

I use Personal Capital to have a unified view of all of my accounts, but I do not really look at the 401k balances that often.

I probably look at mine 1-2 times per year, I check it online and update my personal balance sheet. It is mostly S&P and a bit (15-20%) of international. I rarely (every 5 years, maybe) re-balance and I contribute the max. I don't feel the urge to check it more often.
