Wow. I just got ours in the mail today. It has all twelve months and little boxes that are checked that indicate whether or not my husband and I had health coverage for each and every month of 2015. I have to report this on my income tax form this year.
WTF? I feel like my privacy has been violated by Obama Care.
Still waiting. I am on Medicare. I made an appointment to have my taxes done and the person making the appointment told me to be sure I had one of these. It's two days until the end of the month and it isn't here. One would think the government would make sure everyone would have theirs one time if they are demanding they have one.
It seems redundant for Medicare users to have this anyway. It's clearly stated on our SS statements we receive at the end of the year. Talk about double the paperwork. Wonder how much this costs us taxpayers.

Employers who pay all or part of the healthcare have until March 31 to provide the form to their employees. It's not required to pay your taxes, however, in fact the amount your employer paid for your healthcare is on line 12DD of the W-2.
Got it off the Medicare website. I was told by the tax preparer's appointment person it was necessary to file this with my taxes. It seems like a waste of time since the government is the payer and as mentioned, they are the ones who are paying for my health insurance. They already have record of there being no other coverage other than Medigap which they partner with for all Medicare recipients. So for all practical purposes, I think for those on Medicare, this is a waste of time and paperwork.
I got mine. It shows a checked box for each month I had insurance so I got 2 separate forms because my health insurance runs from October through September.
So here's a question. What happens if you're out of work for a few months out of the year? And out of the 12 months only 9 boxes are checked.
I got mine, but it doesn't have any of the boxes checked although I had FEHBP Blue Cross/Blue Shield all year and Medicare for the latter 6 months (the Medicare info is not even listed at all.) So far, trying to get the info corrected by OPM has not been productive. BTW, according to the IRS website, the form is NOT to be filed along with your taxes, just kept with your backup papers for proof if you are audited.

Although if you know you had insurance all year you should not need the form. The preparer should check Full Year Coverage on line 61 of Form 1040.
