Need a blank subpoena form
Hi,
Does anyone have or know where I can find a template for a subpoena requesting production of documents (ie. payroll info, employer benefits manual, and bank statements)? I don't want someone to appear in court, only to subpoena my ex's information. I am searching on maricopa.gov right now. I cannot afford an attorney and do not qualify for free legal service. I can fill out the forms myself, but just need some help finding the correct form. Thanks a lot!

This is for child support modification. Attorney's do it all the time. I am respresenting myself and need to obtain payroll records, bank statements (cause the ex might have two jobs), and employee compensation outline so I know the amount of commission being paid. I found a subpoena to request someone to testify in court, but I do not want that. I need them to produce documents.

Maxpower123 is correct. Only a court can authorize a subpoena and enforce it. Perhaps you can fill one out, but you will need to have court (Judge) authorize it.

That is correct. Presenting a fake document, pretending to be the court, may offer you a criminal charge. Be careful.

How do I get copies of my ex's bank statements, and copies of checks? They were able to get copies of my statements for the last 6 months.

Who is "they"? And how did "they" do it? I suggest you do it the same way -- check your case file at the court, or, better yet first, ask your bank.
Lots of lawyers will give you a free consultation for a half hour. I suggest you take advantage of that.

Actually you don't need to serve the other person's employer. I just found it on the Maricopa site. They have a template letter that the employer must respond to within 20 days. You can only use this letter once every 3 months. It is under the self help section. Thanks for all your help.

Perfect! I looked on the self-service center site, but didn't get into the packets far enough or didn't see that. Glad you found something that will work!
What about the bank records? How were your records obtained?
