I have an app on the iPhone (called 3D Camera) to make one but you can do it with any camera. You would take one picture then physically move the camera to the right a few inches, be careful to not turn or tilt the camera. The camera must stay at the same plane as the first image. Then take both images and in a photo editor put them side by side, print it out and check it out, if the effect isn't quite right try switching the images, in other words take the one on the right and move it to the left, reprint and check again.

Funny enough, all the wooden grave markers are fake. They were added for an episode of Gunsmoke that was filmed here.
