Would someone, anyone, like to provide some specific examples to actually discuss?
The school should have called the police and had a protective order of some kind to protect the teachers and students who were afraid of him. There should have a been some kind of police report made then so if he went to stalking someone or trying to get back at someone because he was kicked out, there would have been some protection of the innocent.

A drug arrest is another opportunity to look at priors. Often someone gets charges dropped AFTER checking for other events and actions. If it's one drug arrest but the suspect is in school, holding down a job, negative drug test and has a clean history, then dropping the charges makes sense.

Obviously there's a lot coming out about him now, why didn't they discover any of these things when he made a death threat or was arrested with drugs? At least the school expulsion should have been a red flag.

Looks like the Pima County Sheriff Department had legal authority to take Mr. Loughner in at any time after he made his first death threat to have him evaluated but simply chose to not do that.
