Is there evidence that this specific action was done on purpose? Couldn't it have been faulty intel and a natural police reaction to facing a gun in a home they were told held a dangerous person?

I guess verifying the information would've been to difficult and way too much to ask huh? Reliance on CI's results in tragedy more times than the cops would like to admit.

Placing the blame on those that cultivated the CIs and tasked others to act on the bad intelligence would be far more productive than blaming the SWAT officers.

While you're at it assign some blame to the non-LEOs that were instrumental in lowering the standards required to obtain a no knock warrant.
