Ins co have to cover more things now than they did before so, of course, the premiums will go up (not arguing about whether or not they should)

Also, not to mention the fact that now everybody, regardless of risk, must be covered, not a small thing when it comes to pay-outs.
The Mangiones's insurance company, Humana, declined to comment. Humana did, however, include the following explanation in the rate spike announcement letter:
If your policy premium increased, you should know this isn't unique to Humana—premium increases generally will occur industry-wide. Increases aren't based on your individual claims or changes in health status. Many other factors go in to your premium including: ACA [Affordable Care Act—also known as Obamacare] compliance, including the addition of new essential health benefits.
That's why it's not going up, in some states like NY and Cal. the state already mandates the insurance company covers XYZ. In other state someone could get lower cost insurance because it only covered X such as catastrophic illness, emergencies etc. Those plans have been outlawed and those people will be paying more, they'll have more coverage but that doesn't mean much if you can't afford it.
