We ALL should pay for it. Removing a profit making middle man.

Fun fact-almost every quintile pays a combined state, federal, and fee rate of about 20% of their earned income.

ergo pretty much everyone that possibly can has some skin in the game.
Do you have a link that has the numbers to show this fun "fact"? I doubt it.

A alternative would be a flat tax of 17.9% on all income without any exception. And if we went about it like Canada we could probably drop the cost into less then 10%

Oh wait....did you just post this to whine, or were you actually looking for comments?
Yeah, like liberals would allow a 17.9% tax increase on the poor and a tax reduction to the rich.

Health rewards cost-savings -
How about if you exercise healthy habits you pay less out of pocket and Govt picks up more % of the tab. You choose your life habits and can turn down rewards programs but then you will choose not to get the discounts and pay more yourself. This wouldn't be violating anyone's personal freedom. You have the right to refuse health rewards incentives programs.

Then it should be managed like the fire department. Are you prepared to have hospitals owned by the government?

So when do people on welfare pay for it? When do they pay taxes, when do they pay their own way?

Capitalism is set up to most reward skills which are the most rare to the benefit of all. Any conversation about redistribution of wealth is Communism and entertaining these notions means dooming your society to failure.

Wrong answer. Higher income earners pay a grossly disproportionate amount of the federal tax revenue already.
