The ACA grants states the right to include beyond the " Essential Services" .

Some states allow adult children to remain on their parent's plan beyond age 26.

Some states allow insurers to exclude outpatient surgery.

The biggest differences are which medications must be covered.

Regional utilization ( claims made) and competitiveness / lack thereof of regional healthcare providers has a huge influence on premiums.

There has never been a plan that did not change over time.

MDs and hospitals opt in/ out of networks all the time.

Employers were and remain free to change plans every year.

Politicians are not insurance experts.

That's obvious when you hear politicians talking about how they are going to wave their magic wand and allow insurers to sell healthcare insurance across state lines which is being touted as increasing competition and lowering premiums.

Nothing in the ACA precludes insurers from selling across state lines. 6 states allow it. Thus far there are no takers. Each states regulates in state insurers and decides what services/ medications must be included.

Many people, including politicians, continue to assume all healthcare insurance is alike.

Pay it. What are my other choices? Fortunately I'm in a position to do so, but I'm aware that many others may not be without making significant sacrifices or hard choices on what goes unpaid or skipped altogether. Oh really? Pre existing conditions are now covered. Scores of people benefit from that. And if your plan sucks, change it instead of complaining incessantly.
