This is the usual liberal false argument. "Why, some people are born with a costly health condition!" Uh ...yeah? Also, some people are born to rich parents and some to poor parents. Also, some people are born in America and others are born in the Congo. Also, some people are the kids of liberals and are killed before they are born. What's your point?

There's nothing in free market theory that requires pricing transparency. It likely increases efficiency in a free market but plenty of freemarket systems have opaque pricing.
Prices are known beforehand.
You can have insurance or not. If you have insurance, prices don't change (meaning, you can't bill people more with insurance, as they do currently). You can choose to not use your insurance (similar to car insurance) and pay out of pocket or not. If you do not have insurance, nobody is compelled to provide care for you.
If they do provide care, it is considered charitable. Therefore, you agree to either allow trainees practice on you and/or you don't get to sue except if it can be shown that deliberate and purposeful wrongdoing was performed. Medical mistakes cannot be sued for.
That is never going to happen. We are not going to allow people to just die because they can't afford to see a doctor.
They should be triaged and sent home. No one has a right to have a heart specialist flown in to save your life. Make them comfortable until they die.
You can wish it but it's never going to happen. It's a waste of time trying to make your fantasies law.
