Do the countries that have American/UN sanctions against them even care? (Iran)

There are lots of countries that have sanctions; North Korea, Iran, Cuba, etc. but honestly does the ruling power in such countries even really care? To me it seems as if such sanctions only end up hurting the poor/lower classes than the ruling class or elite, and since they don't give a crap about how poor or hungry they are yet still manage to get nice food, nice cars, nice luxuries themselves is it really that big of a deal?

At least it seems to me when I hear that there have been sanctions placed upon countries by America or the UN it hardly seems like it really matters.
