The English Channel has rues of the road that apply and radar, perhaps other parts of the world should be using such techniques.
The Strait of Malacca is one of the world's most traveled shipping lanes and very much has traffic separation and a top-notch Vessel Traffic Services system. (Making, I might add, the hacking/jamming theories even more unlikely. It's probably one of the places in the world where a ship's position can be independently verified to the highest degree.) The rules of the road are pretty much universal.
But - it's still the Master on board whose job it is to conn the ship.
I would think the Ops Department on the McCain will be under a microscope. How did they get that close to the tanker? Was all of their radar inoperable?
