Also with this being the first run of this route for this train I've seen reports that a supervisor was on board as well. So highly doubt this was a texting/sleeping issue.
The number one cause of operator error is complacency. Supervisors are not immune to it either. I have seen plenty of supervisors off coking and joking instead of supervising. Just because someone is a supervisor doesn't make them good at the job. It just means they are called a supervisor.

PTC is not the cure all you think it is. Too much can go wrong with the gear.

When PTC is eventually put on all trains, and it later malfunctions and a train crashes anyway, who will be blamed and what will they do about it?

Here's a stupid question from me... was there any safety railing on the overpass? I didn't see any.
