I agree, but how would that be realistically enforced? Obesity is readily apparent, but poor lifestyle habits can be concealed and lied about.
Given 75% of us are overweight- obese, it is the single greatest healthcare risk. A simple waist measurement is the best assessment.
If we could wave a magic wand and right size the population, the incidence of heart disease, some Cancers, Diabetes, stroke, high BP and joint deterioration would substantially decline, as would the cost of healthcare, thus insurance premiums.
Smoking, substance abuse and reckless hobbies are secondary and beyond risks.
Albeit anecdotal, I know several obese people who have been unable to find a surgeon willing to perform joint replacement surgery unless/ until they lose X pounds. It does not appear surgeons expect perfection. Rather it's more about a 300 pounder losing 50 pounds so they are more able to engage in the physical therapy process, following surgery.
I have no idea what right wing R's want, but I know I prefer to go back to the days where govt stayed out of it and insurance wasn't a necessity to pay a medical bill.

In the US:
Healthcare insurance is a relatively modern construct. Hospitalization insurance however, goes back 100 years, decades before Pennicillion and vaccinations against Polio. People typically landed in the hospital to die.
Labor shortages in the 40's compelled many employers to offer an insurance benefit to their employees.
Medicine, just 50 years ago, was primitive by today's standards. Survival rates were dismal by today's standards. Things that were not even diagnocable and usually fatal are now treatable and often curable. The good old days of healthcare were not so good.
Then came the HMO. The Nixon Admin advocated for HMOs. Congress passed and Nixon signed legislation requiring employers who offered insurance benefits to employees to include at least one HMO option. To compete, traditional insurers expanded their plans to include general healthcare.
Not sure most folk want to go back to a time of relatively primitive medicine when people died of a simple absessed tooth.
I dont see anyone advocating against any of that, its about empowering politicians and bureaucrats to ration healthcare based on their whims and those of their cronie$.

Whims have nothing to do with it. Cold, hard numbers are the reason - both actuarial statistics and patient outcomes. I'm not sure why you are so eager to see your insurance premiums and your taxes go up to provide elective surgery to medically iffy individuals, but oh well.
FTR, I'm overweight myself. So this is not a case of a skinny person sneering at fat people.
Government healthcare rationing will only get worse. Big Government people love to tell others how to live their lives. The UK is a leader on that front.

The restrictions will apply to standard hip and knee operations.

Before FDR decided to freeze wages, and employers decided to respond by offering health insurance as part of the workers' compensation to get around the wage freeze. Government interference is the reason we're in this mess.
