The school where I taught had a mid-90s graduating class that was involved in one. I was there for 25 years and that was the only class involved. The kids told me it started in 2nd Grade or so and they did it every couple or three years.

That would explain why nearly no one has had any knowledge of these surveys. I started school in the 60's, had kids and grandkids in public school and no such surveys were ever given.

Then again, as someone said, there may have been notification sent home and the parent didn't see it, ignored it or the kid just didn't give it to them.
You'd be amazed at the number of parents who come in in May to discuss why their kid is failing who claim to not know there were 4 Progress Reports and 3 Report Cards issued already and to have not seen any emails or letters sent to them.

Letters were sent home according to the mother. She didnt see it or get it until after the survey. I agree with her that this opt out and implied consent is wrong. Like her I have to sign giving my permission for everything else, sex ed class, trips, to watch movies, taking photos, walking or riding a different bus, allowing random drug tests, etc. If there is no signed form there is no participation. So why is something like this different.
As well according to the mother the letter led the reader to believe the survey was about domestic violence and abuse not sexual relationships and gender identity.
