The logistics involved with keeping every single city in every state fully supplied is something that cannot happen totally under the radar, this would be like UPS trying to run its business without attracting attention..its just not possible.

In the US, gangs, thousands of gangs, control the retail sale to the end user.
The dealer has no idea the ingredients of the product be sold on the street. It is not in the dealer’s best interest to kill off reliable daily repeat business.
Heroin has historically been cut with other products at the distribution level. Baby powder, flour and sugar are common historical additives at the distribution level.
Synthetic opioids ( Fentanyl and Carfental) are cheaper and easier to smuggle into the US than heroin. Reportedly, China is the primary source.
I doubt there is a heroin addict in the US who is not aware of the very real risks associated with potential additives. That it does not seem to discourage use is evidence of the power of addiction to opiates/ opioids. It rewires the brain to protect and sustain addiction.
Though I personally agree with what I think the underlying premise is, the verbalization and idea as put forth by Trump is vague, lacks substance and is not well thought out.
#1 is, what right does Trump have to enact or enforce laws in other countries.
#2 - doesn't US law, customs and the DEA already have jurisdiction and procedures in place to control fentanyl coming into the USA
#3 - what about carfentanil which 100 times stronger than fentanyl
