You obviously haven't been to many places in the USA. I winter in Florida and I've seen whole neighborhoods here in "wealthy" coastal areas that are worse than I saw in Jamacia decades ago.
Surely you know that tens of thousands in Appalacia travel for hours and stand in line overnight outside big tens to get volunteers to extract their teeth??
Some of the waiting rooms and operating rooms are literally in pigpens (at fair grounds, etc.).
If this is 1st world, I'd like you to explain why and how???
