15% of gross wages to a person who is earning $7.25 an hour is a hell of a lot more of a burden than 15% is to a millionaire. The millionaire will still live very well on $850,000 while the minimum wage worker will be left with $12,818 to feed clothe and house himself.
Irrelevant. A flat tax is a fair way of assessing everyone for the cost of the Fed Gov. Minimum wage workers must simply live within their means and not feel entitled to anything more than what they've earned for themselves.

So no country with a national health care system has a system where people can pick and choose what they want. Why do you think that is? If you didnt have to pay Medicare taxes, you got sick at an advanced age and didnt have the money to pay for it, what do you think should happen to you?

The only developed country that has working private insurance based model is Switzerland. Basic premiums are FIXED. Insurance companies either learn to leave within their means or else. Mandate of course, otherwise can't work. Hospitals are heavily regulated, etc. No person pays more than 8% of the income towards to basic health insurance premium.
It is somewhat more expensive than the mostly single payer models in other countries, but still reasonable in relation to GDP. In contrast to the situation here.
