In Europe, you have the fredom of getting health care insurance, paying out of pocket, or relying on the health care your tax money provides. In America, you have no such freedom. You pay more in tax for government health care, get no care in return and out of pocket is far more expensive than in Europe.
Elsewhere, because healthcare is not a function of your employment, you never have to make the choice between your dream job and one with health care.
You can have kids when it feels right for you as a family.
You can start a business of your own when you feel you've gone as far as you can working for someone else. Without worrying about healthcare for your family or youself. You can take a year off for personal reasons if you can support yourself.
You can retire early if you got the money.
The sum total of those things - pursuing your dream job, having kids when you want, how much free time you want to take, going into business for yourself if you fancy, when you can retire- the difference in freedom is monstrous!
If you are young, you may not yet have experienced how important your healthcare becomes in those decisions when you age, and get dependents. And how incredibly restriced your choices are because of it.
Well considering there wasn't much of a "health care" type structure back all those years ago.. I think it's safe to say that it's something they never accounted for..
But.. if we're all entitled to "life" and lack of healthcare = death.. well you get the picture.
Healthcare consisted of a man with a few elixirs.. maybe some leeches and prayers.. we've evolved since then.... healthcare and access to it wasn't even a thought then!
Nor were there health insurance companies that bleed you dry either
I think that the problem is not the quality of the healthcare in the U.S but how much the insurance companies have in the amount/type of treatment the hospitals/physicians are allowed to use.

'freedom' to be treated by doctors wearing lab coats with body fluid stains from the previous patients. Buckets of sawdust in each room, for sprinkling on the floor to absorb excess body fluids that spill.
'freedom' to eat any food that your family brings in to feed you [because Ospitales do not serve food].
'freedom' to be rationed healthcare by according to your age and education level [unless your wealthy in which case then you go to private clinics].

What I observed was a lack of sanitation.
