Why didn't the Bush administration implement healthcare reform back in 2005/06? (Congress, health care)

This would have been the time to have done it; back when the GOP held the White House and both houses of Congress. It was a major issue, and there were plenty of calls for reform of some type to take place.
Why did Bush ignore the issue almost entirely, and instead pass it off to the next administration? Why didn't the "party elders" in the GOP jump into action once the path was clear?
This is something I really don't understand - why not at least implement some reforms, even small ones, like permitting cross-border insurance plan purchases? Why do nothing in the field of overall health care reform, and thus cede it entirely to the other party?
