If we accept that NPD is the main affliction, how does that relate to all of his lying? Is being a liar part of his NPD, or is it a whole different issue?

He's little different to many other people in his position. Most US presidents are egotistical psychopaths to an extent, that kind of personality is perfect for a position that often involves lying, manipulation, the ability to handle intense scrutiny, etc.
The only difference is that Trump doesn't publicly hide these traits, he openly displays them. Other leaders who be charming in public, but terrible behind the scenes. The fact is, in politics perception is more important than reality. If a politician comes across as nice, and media coverage is generally positive, then that politician will be popular, so long as their track record isn't irredeemable. Many people liked Obama because he's "cool" and seems like a nice guy. Despite the fact that he was neo-con war hawk, the only President in US history to be at war throughout every day of his two terms.
Not atypical for someone with this diagnoses, trump is in way over his head. He sold out to the "religious right" and now he must pay this unpopular, vocal and demographically dying minority back, although he lives about the most worldly lifestyle of any president ever.
Trump is emotionally fragile and exceedingly thin skinned. He has the LOWEST APPROVAL RATING of ny modern president at this point in his presidency, and his desperation to be well liked is pathetic.
While he argues with a department store and "Saturday Night Live" he is decompesting (losing it) before our eyes.
