I guess probably lots of reasons, including perhaps some combination of any of the following. In no particular order of importance:

• Their parents did not train them or model being ethical;
• They are self-centered and narcissistic;
• They lack empathy toward the needs and feelings of others;
• They get a little sadistic thrill from hurting, confusing, annoying or inconveniencing others;
• They aren't very bright or self-aware;
• They have so many personal challenges that it's impossible for them to be very reliable;
• They're depressed, or drug or alcohol addicted, or addicted to some behavior like gambling or sex addiction;
• They're under a tremendous amount of stress;
• They may be disoriented with respect to time, due to medical condition(s);
• They suffer from "the world revolves around me" syndrome

There are doubtless other reasons. It's also important to state that if a person is unreliable, all of the reasons on the list won't apply, nor is it necessarily an indication of a serious personal failing, though it might be. Some reasons may not be their fault. Or temporary.

A really good question!

Some people never learn to be responsible. They never learn to be accountable.

I think it starts with their upbringing and it spirals from there.
