She's 400 pounds and you THINK she has an eating problem? You can either hold your ground in these conversations, or just humor her and let it go. How often does she visit? If it's only a couple of times per year, and your husband enjoys her company, then humor her and have another drink. Does your husband know how you feel about her? Is he in agreement?

it is unfortunate she is ill, but you are not her slave, and in your home you get to set some rules

one could be "We don't feed guests. We love your company but no food is served, so please come after you've eaten or bring your own food to eat."

another could be, "our home is not able to accommodate your health care needs, so let's meet at your house (or such and such restaurant) to visit."

decide what period of time you are able to be with her out of "family duty" and stick to that. Let your husband know, "I am willing to visit with sister 30 minutes once a week." Beyond that, if he wants to deal with her, that is his choice.

The advantage of meeting her somewhere other than your home, is that you can come and go as you please, you are not a prisoner to her demands.

All this can be done in a pleasant way, "We would like to see you" "Yes let's get together" without you being held prisoner and made miserable in your own home.

If hubby doesn't agree, let him spend time with sister without you having to deal with it. He's an adult.
