This is a weird question perhaps but you'd save me grief by answering. My ex works there and my kids are on his insurance. We haven't spoken to him in years. I known the health insurance is Anthem Blue Cross but can someone let me know if the same is true for vision and dental? If not can you please let me know what company is the carrier?

I worked at NetApp in the past. They had a few different options for medical. PPO's and HSA's. I do not remember the provider. Vision was VSP. I forget what the dental was also. There were a few different options for dental.
Perhaps you could call NetApp and find out who manages their benefits (Fidelity?). Then call Fidelity to get the details of the plans for your kid.
Or email your ex to request the plan info. The kids may have no coverage at all - better to find out now than later.

You should have OLD insurance cards from Anthem, correct? Contact them FIRST. As an insured, they WILL talk to you.

If they don't have you listed or are not the employer's insurance company anymore, you can try calling the employers HR benefits department. If ex husband's coverage enrollment forms list you, they should be able to discuss this with you. If they list only the kids, I'm not so sure.

Can't find anything? Either bite your lip and contact ex husband or find a lawyer. Probably best choice would be the one that wrote the divorce agreement stipulating that ex has to provide health insurance.

A personal suggestion: mentally and financially prepare yourself to find out exactly has not covered you and/or kids. I say this because you evidently haven't received new ID cards from an insurance company in who knows how long. Most if them send new ones every year. If ex doesn't have your current address, get a PO Box and give it to him.

"He's required to carry the insurance." Unless he is specifically required to purchase Dental and Vision in addition to Medical, he (and they) may not have it. Many employers automatically provide Medical, but offer the other two as optional "add-ons" that the employee must elect & pay for separately.

Dental is through Delta Dental (PPO or Premier)
Prescription Benefits are through the PBM, CVS Health (aka, CVS/Caremark)
Medical is Anthem BC/BS or Kaiser (if in California)

The Anthem plans are the:
• Health Savings Plan, a high-deductible PPO linked to a tax-advantaged health savings account (HSA).
• Traditional Plan, a conventional PPO.

Kaiser
• If you're in California, you may have to get Kaiser insurance
