The worst I heard of was a guy pretending to have died in a car crash and his girlfriend was so traumatized that when he woke up he asked her to marry him and she said yes. When asked, he said he wanted her to know that she couldn't survive without him and to feel so devastated by it that she'd have to marry him.

I would not want a huge crowd of people around. While it could be fun to have people cheering when the lady says, "Yes", it's still personal - between two people, not a crowd . . . but that's just me.

We might think that guys who propose to their fiances at baseball games are really sweet, but I actually think it kind of puts her on the spot. I mean what if she says no? People are gonna think she's a...well a party pooper, at the very least!
I have helped with several proposals and even marriages in my line of work. No proposals were bad, usually horse drawn carriage, restaurant, landmark etc. One a few years ago stood out. A guy created a proposal on the Times Square titantron that I helped with. That was cool. She said yes. I actually forgot that one til I read this thread.
I think the worst would be proposing in McDonald's or a fast food place.
