I don't think most lady friends would be too excited about such a gift. I wouldn't make the offer until I had a strong feeling that she'd like to have it done, as it could seem a little off to some. If you do it, select the photographer very carefully, and spare no expense. I am, or was, a professional photographer. You wouldn't want me, as it was never my specialty, and I'd not do a good job of it, plus my small studio wouldn't work for that type of portraiture. (I was asked a few times, however. Always said no.) The best ones specialize in it and usually have their own beautician on staff, if only part-time.

No definite answer. Women I've been involved with in my distant past would have been all over the board as far as answers. From "I'm so flattered you think I'm hot enough to do this" to "what kind of hoe do you think I am?" Unless you know her really, really well, put out some feelers, don't spring it on her as a complete surprise.
