I didn't do the poll because there really wasn't a choice that fit. I live in Las Vegas and we eat out a LOT. We get freebies or at minimum BOGO's from quite a few casinos every month. We eat out a couple times a week and usually it's free. We don't cook much in the summer because it's so hot. We do cook more in fall and winter when it's cooler.
I was waiting for someone in Vegas to chime in. Good thing I don't live there, I'd weigh 400 lbs and remove the kitchen from my house as there'd be no real need for one.
Have you been to the new buffet at Caesars Palace yet? I hear it's topped Bellagio as the best in Vegas.
There are only a few restaurants within 10 miles of us, so we have picked the ones we like and eat at them all once or twice a month, so that works out to having lunch or dinner out about 8 times a month. Mostly they are sit down restaurants, and occasional pizza or fast food. For really good fresh foods with interesting preparation we would have to drive 20 miles or more, so we go to those only when we are in that area, or for a special occasion. It's just not worth a half hour drive each way, I can cook faster than that! When we are in a hurry, or want to sit outside, the golf clubhouse is about a 4 minute walk from our house, and they make good sandwiches and burgers.
Until really recently I cooked mostly for myself. Lately I've been doing more take out. Discovered Chipotle's. I think its good food even if it is a chain. A lot of Panera's. A small steak meal from Texas Roadhouse or a philly cheese steak from Penn Station. Its mostly chains near me and I'm not driving halfway across town and back for takeout, so chain food take out but usually a small step up from Burger King.
To me it is not dining unless one can have a drink. Otherwise it is just eating. Rarely do fast food unless on the run and hungry. As I like about two drinks before dinner and what with the drunk driving laws and liquor costs, we rarely dine out anymore. We do takeout about twice a week. Usually Chinese and a local down home cooking type place. Down home place has a Sunday Pot Roast special which is more than likely what I will takeout tonight.
We eat breakfast out 3-4 times per week but usually about 10am or so unless I am playing golf then I go by myself at about 8am. Thus breakfast is the meal we out most of the time.
I'm not a big dinner fan but I love going out for breakfast and lunch.
Denny's is hands down my fav breakfast place and non-chains are my fav for lunch.
And every once in a while I get the hankering for a McD's sausage and egg mcmuffin.
We tend to eat out more than we'd like mainly due to schedules. The bad part is here locally there are very few options that aren't either chain fast food or lower end chains. We don't even have a "higher" quality chain place. There's really only one local owned place that does the true fresh high quality food (owner is classically trained chef). The others are of the typical "you should open a restaurant" variety that are one step away from being on Restaurant Impossible.
Heck, where we are most folks would love to see even an Olive Garden, Chipotle, or 5Guys as a major step up from what we have available.

I avoid most chains and find their food tasteless and also I am not sure of the quality of the ingredients. So when we go out I like to eat only in places that serve fresh food, well made. and tasty. They are also a bit expensive and my DH balks because he is a vegetarian, very picky, and feels what he gets off the menu is overpriced. it has become no fun anymore to go out.
We rarely go out to eat because we have found that homemade usually tastes better to us and is healthier. When we do go out, we have a few favorites. Breakfast at our local diner, an excellent Chinese restaurant, and a wonderful authentic Mexican restaurant. We also have a very good deli within walking distance.
When we travel, which is quite often, we always look for local places.
We usually just go to the local fairly good places. We used to enjoy more "fine dining", but those places no longer seem so "fine". They are noisier, seating is crowded, and the food fairly unimaginative and too fattening.
These polls could stand a lot of improvement. The poll asks, "Where do you eat when you go out?" then three of the options don't mention the "where" but the "when." My answer to OP's questions is my wife and I have several standby restaurants we go to, but oftentimes when we're tired of those, we get a Groupon for a new place, try it, and if we like it, go back when they offer another Groupon. We go out once or twice a week. Our usual standbys are Whataburger, Chick-Fil-A, Smashburger, a local Chinese Buffet, a local Indian buffet, a couple of Mexican places, a nice Brazilian Churrascaria, and occasionally we head to Applebee's for their cheap LIITs and Bahama Mamas.
