Not that this really makes SA "small" but this town has never had a Modern Rock Alternative station (don't say KISS because it's not) like 99% of other big cities do. Even some mid-size towns and smaller ones have em. Examples would be The Edge in Dallas-Ft Worth, and 101X in Austin.

It's a combination of things. The scale seems small: Not a lot of wide boulevards except the major commercial streets. The wealth isn't glaringly obvious, so what well-off areas there are don't stick out as much. Multiple generations of people stay close, with family friends marrying and living near relatives. For all the freeway-close chains, there are a lot of local restaurants and shops that cater to the masses. The airport is manageable. The Spurs are collectively popular.

It'd be nice for somebody to explain exactly what the heck gives San Antonio a small town feeling.
Because it sure as hell is not a small town when it takes me almost 1 hour to drive across the city.

Another possible reason,
the malls look like small city malls (small)

There's not one answer that one could give to answer it. San Antonio is just a big place that groves like a small city.
