If you have a primary physician have him/her write you a referral for one. From what you've described your vision is changing (likely due to age) so you'll need the full one where your pupils are dilated and a picture is taken of the retina.
I was in the same boat as you, 20/15, until I was in my late 40's when I noticed I needed cheater glasses to read newsprint and for close work.
I hadn't had a full eye exam since my final flight physical 30 years ago until last year when my doctor finally laid the law down.
