This happened to me many years ago. I was baking a casserole to take to a covered dish affair, and when it was almost done, I heard what sounded like a muffled shot from the oven. Opened the door and the casserole was everywhere along with broken glass. Needless to say, I was hotter than the dish.
I have many pieces of original Pyrex, use them often, and have never had this problem.
