How do you feel about being asked to complete a survey when you go shopping at any type of business?
How do you feel about being asked to complete a survey by a supervisor or a manager if you feel that you are a highly satisfied customer and to let the business know this?
I listen to the spiel then don't do it, because they want my email address. I also make up answers on surveys so I can read newspapers online. I guess quite a few people take the bait but I'm not one.

What if the survey is on the receipt you get, and you have the option of not giving out your email address?
Unless its's a total screw-up, I always give perfect scores when rating in-store or call-center employees. Anything less than 10 is used a club against employees - even if it's a decent 9/10 they still get flak from management.
I have a spam email for this purpose and I'll usually take a couple minutes to answer.
I have earned some pretty good discounts through my insurance company, credit card, and a few other companies I have a card/membership with doing this.
Surveys benefit everyone. Most businesses and customers depend on them.
Honesty is important, not making others feel good or keeping out of trouble. Surveys are important for improvement.
I always do the survey on the Smash Burger reicept because then I get free fries. If there is a guaranteed incentive, and its a place I go to often, I'm more likely to do the survey. Plus at Smash Burger, I always do the survey while waiting on my food, so I'm also more likely to do them when I have the time.
But places where they claim that filling out the receipt will get me entered into a yearly drawing for a $10 gift card? Not worth my time.

They should have a complaint department if they want honest feedback. Surveys with generic questions may not truly address the shoppers experience -- either positive or negative.
Surveys are done for a number of reasons. Some, like the ones you get in the mail for the "save-the-tree" society, are all about getting an interest in the agenda and names and number so that money can be sucked. Store surveys and customer service surveys are a low cost alternative to an undercover agent or secret shoppers. The random "prize" may or may not exist, and most of the survey results will get tossed, or skimmed by a middle manager and then tossed. Spending time on a survey is working for the company for free. If someone came up with a survey that included a 25% discount on my next purchase - with no strings attached - I might bite. Otherwise, you want me to work for you? Pay me.
