I have disassembled, moved and reassembled several pool tables. They are easy to take apart and I have a few tips I can share.
Crawl under the table and mark each corner with chalk, this will let you match everything back up once your ready to reassemble. I use a different color of chalk for each corner and mark the legs, sides, brackets, etc.
The rails will unbolt from the bottom, you just need to get the right size socket wrench. As you take off each rail wrap it in an old quilt or blanket to protect the finish. Your pockets may be stapled/attached to the sides so make sure they are free before you lift the rail off.
I am guessing you have a 3 piece slate, most are. Be very careful handling it, it is very heavy and if dropped could break.
The bolts for the rails go thru the slate and once the rails are off the slate is just setting on top of the frame. This is when you will go around and pop the felt free (staples), you will want to roll it up, do not crease it. The seams between each section of slate will be filled with bees wax or something similar. It will pop free when you lift the slate, don't worry about that as you will have to melt new wax in when you put it back together.
I lay down a quilt, put a piece of slate on top, another quilt, next piece of slate, etc. Make sure that whatever you put between/under the pieces is very flat.
Once you have the slate off the rest should be self explanatory, it's just unbolting the sides, cross members and legs.
Put all the bolts and washer in a zip lock and you are all set, you should also find a few shims which you will want to hang onto.
Forgot to say, storage should be roughly 50% relative humidty, 50-75 degrees and DRY! When you get ready to reassemble bring it in the house and let it set for a day or three first.
