It depends. I've had to have my doctors fill out a **** ton of paperwork for me for this leave. Both have had to do it multiple times due to "lack of information".
That paperwork is supposed to go to the Principal first before it gets kicked upstairs.
Turns out that even though we can accumulate sick leave they don't expect us to take it, at least that's what I was told the second time the paperwork was rejected.

What I meant was that the principal does not need to be told that the teacher is in an alcohol treatment program, or is in psychiatric hospital, or is in HIV/AIDS treament or has cancer, or the specific medical problem,
I believe that a teacher has a right to medical privacy just like any other person.
Why would the principal need to informed about personal, private medical information?
That is not the way it works in my school district. All the doctor needs to do is state in writing that the person is unable to fulfill the duties & responsibilities of a teacher and any available sick days are used and then long term disability starts (after 60 calendar days). Of course, if you do not have enough sick days it becomes unpaid time off until long term disability starts.
Frankly, most principals can not keep anything private. I certainly would not want my private, personal medical information to be told to him/her. I could picture at least one of my former principals "accidentally" putting it in the school newsletter or telling the PTA. Yikes!
