I am not a fan of Healthgrades.

I'm sure the Cookeville hospital is very nice but to compare it to the above is ludicrous.

The truth is, most every hospital that has a semi-full range of services can reference some ranking that puts them in the elite in the country. The small print is that these rankings use unproven datasets when it comes to quality as well as subdividing hospitals into so many categories and classes that they give "top" awards to dozens of hospitals in the US. For example, they might have a category titled, "community, nonteaching hospital 100-200 beds" and then another category, "community, nonteaching hospital 200-300 beds" and then another category "community, nonteaching 400+ beds." It is entirely possible that a hospital be ranked first in their category, but if they were in another they might not even make the list.

In general, ranking medical centers is incredibly tough to do because no one knows definitively how to do it. Even using metrics like mortality rates, infection rates, and complication rates don't work perfectly because the patient populations vary so much from one hospital to the next.
