Once, a very long time ago when budget airlines were first becoming a thing in the UK. A friend and I walked up to the EasyJet counter at Luton airport and bought tickets to the next flight, which was to Girona. Then we had to ask where Girona was (it's about an hour outside Barcelona). They had all the amenities of a flying Greyhound bus but EasyJet and Ryanair used to really provide some spectacular deals back in the day.

Yes, but only when on a private plane.

You need to be pretty wealthy to be that spontaneous. Either that, or you avoid the international terminal when you do this.

Once on PeopleExpress (you paid on the plane), Just show'ed up and went on last minute trip to Denver.

Drove a Church friend to who was moving to Kokomo Indiana, Had reservation, but no ticket to fly back from IND to EWR.

When I was on 6month loan to a job in Houston, we could fly home every other weekend. But my Townhouse Mate and I would often go to IAH, and just pick somewhere else to go to and change the ticket.

The company I worked for had a deal with the airline that we were charged by SeatMile, So accounting did not care where we went as long as it was ~1250 miles or less.
