Default While Flying, I had a bug "inside/inbetween" my double paned window

Tell me your odd occurences while flying? How does a bug get in between an aircrafts double pane window? Think about it? That is a major breach of safety and security. I hear that a window just blew out and a nurse got halfway sucked out of it.

The sidewalls are fiberglass panels. The part of the window that you actually put your nose against is a thin piece of clear lexan. It can be easily popped off and changed. The real window is a few inches away, and very thick. It has two layers that are a 1/4" thick each. It's easy for bugs and dirt to get in between the lexan, and the outer window. I'll have to do some research before I comment on the above window failure. This is the first I heard about it.
