I've always been on the lookout for something, but the closest I can find is Disneyworld packages. You can purchase a meal plan to use in the parks and at restaurants on the grounds of the resorts. The best option is probably a cruise.
There are also ranch vacations that are all inclusive. They are mostly focused around horseback riding (for the whole family) but there are usually opportunities to fish, hike, do rodeo type activities and even take overnight rides. Most good ranches have kids programs during the day so eveyone gets to do their own thing and then hook up at dinner for a big meal and campfire activities.
An all inclusive resort would be cost prohibitive to operate in the U.S. with our higher wages. It would cost way too much. I've never heard of any all inclusive resorts in the U.S... I would say just go stay in a Great Wolf Lodge and eat at Golden Corral. lol.
Dude ranches, cruises, and Disney are the only ones I can think of.
All of the resorts and the National parks have restaurants, so it is not like you can't get fed. Lots of motels will include a breakfast, of sorts.
The national parks have OK restaurants that don't gouge the tourists, so they would be a good option. There is certainly plenty to do at a place like Yellowstone or Yosemite.
There are tons of them. And they are not dude ranches or Disney, but rather beautiful resorts.

Those are a good start, I know there are probably 20-30 more.
They do exist. The problem is they are so much more expensive than all-inclusives outside the US. We did this one for a family reunion a few years back:
