I regularly get this discomfort but one time from Paris to Berlin my ears sustained some kind of damage where I was miserable for the first week of my trip. Coffee made it worse and beer and ibuprofen did not help at all. I did all the touring I planned while feeling like crap. When I finished in Copenhagen, I bought some hashish in Christiana and it took the pain away and started the healing, I was fine and didn't need a doctor once home.

Try Otrivin nose spray one hour before departure/arrival and hold your shoulders straight/on the same height and bend your neck a couple times.

I ALWAYS start taking Sudafed a day prior to any flight, and every 4 hours during the flight...otherwise, it feels like my ears will explode....I'll be in major PAIN...not just discomfort!

On a 10 hour flight, my dr. gave me a cortisone nasal spray to keep inflammation in check.

I just came back from my trip a little while ago and to my surprise, of the 4 total flights I took I never once even got a feeling of the sinus pressure I had experienced during my flights in the last several years. I took Sudafed once but the 3 other times didn't. A pleasant surprise. I wonder why this time I didn't get any sinus pressure? I can't think of anything that was different with me than the last times.
