This pretty much sums up my feeling as well. I've gone to places many wouldn't consider clean (Morocco, Egypt, Thailand, Cambodia, Tanzania), but if I really want to see a place I'll go regardless. So far, never had anything bad happen to me. Of course, this is highly subjective too. Heck, there are many people that think Paris and London are dirty.

China is a big diverse country, one cannot make a claim that it (as a single entity) is clean or dirty.

Assuming your cousin went along the standard tourist trail she saw a lot of clean areas. This isn't the case with most of China.

I want the hotel I'm staying at to be clean. Locations like beaches, parks, etc., where people go to relax outdoors, I don't like them to be covered in litter.

Otherwise I'm not too concerned about it. I don't expect large cities to be spotless. Urban locations can pretty much be expected to have some litter or whatnot lying around. It's not as pleasant as a relatively "clean" city, but I wouldn't really notice it enough for it to ruin my vacation or anything.

Haha after wading through thigh-high water in the streets (and bars and shops!) of Pattaya, Thailand, I really don't have a lot to complain about anywhere else. I'm certain it wasn't JUST rainwater from the tropical storm, as it was bubbling up from the drains...but it was fun....and the "events" continued, regardless!

Once you're used to squatters there is something to be said for not actually touching anything.

No matter where you travel it is possible you'll have to use a filthy bathroom, if that happens I'd much rather it be a squatter than attempt hover contortions over a western toilet.

Squatting is the easy part, standing back up is not. Also keep in mind in countries with squatters some of the people before you cleaned their behind with their left hand. So don't touch the floor or the wall on your left side.

I expect urbanized area to have some grime and trash. But filth is another issue altogether, and when raw sewage runs through the streets, it becomes a health risk. No thanks. That's kind of my dividing line.
