I don't know if it was the worst, but probably the dumbest. Which I can look back on and laugh now, but was incredibly frustrating. Back in the 70s a group of us in the service were traveling together on Republic Airlines. Plane was late getting in to our departure airport so we were way late taking off, actually taking off after our planned arrival time in Atlanta. We all had to change planes in Atlanta and were so far behind we were worried about missing the flight. Came in on one concourse and of course the next flight was on a different one. So we did our best OJ impersonation sprinting through the airport with our luggage to arrive at the gate and find it empty. We go up to the agent and he says, oh no, you haven't missed your flight. The plane just got in and they have to clean and turn it before bringing it over here. With that he pointed out the window across the tarmac to the very plane we has just gotten off of.
Crowd probably wondered what was wrong with that bunch of young servicemen roaring with laughter in the Atlanta airport.
Primarily not because of the service but the general public on the plane.

Small children crying EVERYWHERE.
Moms opening the baby poop filled diapers by using the food tray tables as the diaper changing tables - stink and chaos EVERYWHERE!
Air Tran. It's the only airline I would never fly on again, horrible.
You don't have to worry about them, they no longer exist. Southwest bought them out, and sold all of their Boeing 717's to Delta.
I don't really have a 'worst' to be honest. My least favorite of the legacies is United, and I'm not a big fan of Southwest. Living in Atlanta I love Delta, but I've always liked them even prior to moving here. I also have their Amex card, it makes a big difference. I'll fly any of them though if things align to our advantage.
We recently flew jetBlue for the first time, and really liked them. I would really like to check out Alaska, as I have always heard great things about them as well. They just bought out Virgin America, and have become pretty large now.
Spirit is an absolute no go for us.
I still miss several Airlines I grew up on - Eastern, National and Piedmont in particular.
Another vote for Air India, and this is for the plane.
I flew from O'Hare to New Delhi with Air India on a Thursday, and came back on Monday. I expected the service to be crap, but I encountered something worse.
I generally take my seat in plane and go off to sleep even before the plane starts taxiing. On this flight, 5 minutes being seated in my seat, my bums started hurting. So, I moved/ twisted my body. Felt the pain again after 5 minutes, and then it never stopped. After 2 hours, I was feeling the springs of the seat in my bum. Of course I was unable to sleep, and landed in a different time zone completely drained out.
But, this was not the end. The story repeated on the return flight. When I got down at O'Hare and got into my 2006 Camry, the seat felt like a Lazy Boy recliner to me. My bums felt like they were in heaven.
Air India is well known for ****ty service, especially if the color of your skin is not white. I was OK with it as it was just a 15 hr journey and I was seated in economy. But no proper cushions on the seat? That is a bit too much.

I don't need to worry about it as the airline's out of biz... but, it was the plane I was on that crashed decades ago. No fatalities, many minor injuries. And, it certainly didn't help my fear of flying, which I had in a good, heaping dose before that flight.
I avoid flying whenever possible, but need a good strong ale or 2 before I'll traipse down the jetway.
Used to fly Frontier all the time until they changed to the nickel and dime you to death business model. Now I refuse to fly them or any other airline that charges you for everything from checking bags to sitting down in your seat. I'm surprised penny ante outfits like Frontier and Spirit haven't put credit card readers on the lavatory doors yet. Cheap bastages.
For me it's also Spirit. While some of my dislike comes from the fact that they started the whole nickle and dime model by being the first US carrier to start charging for bags and other "innovations" most of my dislike is because they are the most uncomfortable airline to fly, they have the smallest seat pitch of any airline, and things like the seats not reclining add to the misery.
For international carriers, my worst experience was on South Africa Air. I could accept having no wifi, but at the very least they should offer some way for you to charge your device. Also the food was pretty terrible and they had some weird rules like you weren't allowed to have your blanket on during take off or taxi even if you buckled your seat belt oven the blanket
