Where did you go for honeymoon? Is it desirable to repeat similar?
Her favorite romantic spots? DW would kill me if I took her to a city location and called it romantic. (She just at this moment mentioned that New Orleans would be the last place she would want to go again (been 4x in 40+ years). Maybe Quebec City is romantic (a little).
Ideally she would go to Tetons. Or a quiet / off season National Park Lodge. Maybe a snowed in one (like Paradise in Mt Rainier NP).
Plenty of romantic places in Blue Ridge Mtns. Or on Carolina beaches. We found several romantic towns / old lodges in eastern seaboard towns.
Cities for her.
Cabins, rippling brooks, mountains, quiet, fireplace, home cooking... +++
I would recommend keeping the commute to a minimum, unless that time together is treasured by each of you.
Plan your escape through her eyes, as she will be interpreting it. If you envision her smiling and laughing with joy while you are planning... You are on the right track..

If you're a fan of Dirty Dancing, you'll recognize it immediately; it's where the movie was filmed. It's a beautiful old historic lodge built on a mountain, next to a lake. Hence the name.
I'd agree with some of the above suggestions, depending on what she likes to do.
Cities - Nashville, Washington DC and Philly. AND Cleveland. No, I'm not kidding. It's the most underrated city I've ever been to (and I've been to a lot around the world). Great dining, fun downtown where the close the streets, Rock and Roll Hall of Fame (don't have to be a fan of rock to enjoy it), Football Hall of Fame (don't have to be a football fan to find it kinda cool), wineries, some neat boat trips on the lake, and some very pretty places to walk around the lake. Personally, I wouldn't go to any of those places in the winter.
Want really close by? Consider Asheville, Savannah, Charleston - I think ALL of them could be classified as romantic. Maybe the Carolina shores, especially in shoulder season.
A little farther away, but still within driving range - St. Augustine, FL. Another underrated city.
Seriously though, another beautiful under-rated area is the western part of West Virginia, if one likes the outdoors! And it could be easily combined with exploring the Ohio river valley like arr430 suggested.
