Passport -- What Constitutes an Affidavit?? (France, agent, information, places)

Hi. Not even the National Passport Agency can answer the following question. More accurately, agents gave me several different answers.
I was born overseas in 1955 to my married U.S. citizen parents when my dad continued his military service. I've lived here in the U.S. since I was a baby.
One of the stipulations for me to get a passport is:
"An affidavit detailing all periods and places of residence of your U.S. citizen parents' presence in the United States and abroad before your birth."
What constitutes an affidavit? I can list all the information but in what form? Then do I notarize it?
Check to see if DS-5507 would work for you. This is often what military members use for their children born overseas where a Consulate or military certified record wasn't obtained at the time of birth.
You probably need to request a document from the State Dept such as "birth of child of American parent born abroad".
I was born in France and have this document. The parents (Dad was Army) have 48 hours to report the birth for American citizenship purposes.
I recently tried to obtain a new copy as mine is in tatters, they instead issued me a certification that is basically the same thing as the form I had was no longer used.
