Have you tried the Elderhostel experience:

I have never used their programs but I have run into several groups in my travels and the people have been very positive about the programs.

Rather than CouchSurfing, she might be better off using licensed B&B's. The owners are generally very accommodating and knowledgeable, and many B&B's have smaller rooms suitable for a single traveler with rates to match.

Hi! You might want to explore the sunshine in Cebu, Philippines. I am a gerontology Nurse, and we have recently launched an elderly tours to cater the special needs and interests of the elderly. It is the first in the Philippines, and one of a kind because we have a Gerontology Nurse and healthcare assistant onboard. The age range of our guests are from 55 up, as long as you are still physically fit to travel. Just recently, we got requests from groups of mid 40's to admit them to our tours. They were impressed by our itinerary and they wanted to join! We accommodate the elderly and their significant others (husband, kids, grandkids, travel companion) to our tours. We understand the needs of the elderly so we are keeping the tours in small groups, minimum of 6, max. Of 12 guests. Since when is 55 elderly? I'm close to that and run five miles every morning.
