  • proof of age
  • residence
  • gross annual household income
  • insurance coverage (if applicable).
  • List of meds
  • List of your physicians

I could use some extra blood strips each month. BCBS sends out letters telling diabetics what they need to do to stay healthy. They stress that a diabetic should make 7 blood tests every day..........one before each meal, one 2 hours after each and one at bedtime. That is approximately 210 tests each month. What's the kicker?? They will only pay for 100 tests each month on my deductible. I can no longer afford to make 4 tests each day to keep my A1c at 6.5.

Another example of office people practicing medicine without a license!
