What I thought was interesting, is a view of socialized medicine. The townspeople registered with Doc Martin, and they had to go to him, sit in the waiting room for hours, and only see a specialist if Doc decided they needed one. On one hand, the Doc handled most things just fine, but on the other hand, if you had a heart attack or stroke, you are pretty much toast.
Hey, same thing happens with me here in the U.S. I have a GP, and I have to go to him first, unless it requires the ER or an ambulance. My GP refers me to a specialist if one is required.
Now, in a place so far from a larger city, I imagine ambulances are a problem as they are in Portwenn on Doc Martin.
But he doesn't even have a nurse or a PA. All he has as an assistant is his receptionist who has been trained to take blood. I assume, she also cleans up after the patients and is responsible for removing the medical waste.

I liked both the receptionist , there weren't the old run of the mill doctors receptionists , both had their own odd personalities like the rest of the villagers Lol. I'm going to miss that show so bad.
At least Morwenna is better than Elaine was in Season 1. LOL
^Yeah, I was so relieved when Pauline (sure, she was quirky and dressed outrageously, but she was a decent person) replaced Elaine. Elaine made me way too angry (the way she toyed with men was esp. offensive), she seemed to have no redeeming qualities.
Was worried when the actress playing Pauline left, lest the new receptionist/office manager/gatekeeper be abrasive & callous like Elaine, but fortunately Morwenna seems pretty competent & motivated (though she started off timid & clueless).

Before he came there was no local doctor and they were delighted to see him. They had to drive to the next town, not so good when people fish and the main occupations could bring injuries that needed immediate attention. I doubt a town that size in the US would have its own doctor. The former doctor died I think. Nobody wanted the post in a teeny little town. They lucked out getting a London surgeon who had to downsize.
In the episode where Burt gives them the honeymood from the dark side, you can tell how far from another town they are.
The same situation exists in a lot of really small rural towns where there isn't enough population to support a medical establishment and they rely on county services and medivacs.
