Unfortunately, you will always get this kind of foul-up in any system. From the article, it is not clear where the blame resides; with DWP or with the doctors.
In defence of DWP, when my mother passed away they continued to pay her pension until her death could get into their system. They wrote a very nice and respectful letter asking us to repay the overpayment (which we did). That experience contrasted with the terrible experience my sister-in-law had with the VA in America when her husband died and they just went into her bank account and grabbed what they thought they were owed without warning.
