How do I file a notice of intent to defend document once receiving a court summons? Is there a specific form or can a letter be written? Who & Where do I submit this form to?

I would contact an attorney for all legal advice. You will probably get some good advice here, but if its wrong I doubt a judge will have any sympothy.

I've been on both ends of a court summons, a sender and a receiver. There is really no generic answer to the question you asked, unless provided by an attorney. If you have received a court summons, you need to talk to an attorney.
