I would say stay where you are, and try to brush up to make yourself a more desirable candidate for a new job. Either develop your skills, get new ones, get certifications, relevant education, do consulting, etc. And then try to line up other jobs in the event that you are laid off. Also save up money, and reduce your liabilities, like mortgage and car payments.

The doctor's note seems like a red flag to me in terms of showing you what the company may be like. Also, the language you used, that they need help with a bunch of projects, seems like it could be a chaotic environment with lots of extra hours (this is just a feeling, not based on much except your wording and that it matches the hectic smallish environment I work in).
What does your gut feeling tell you? Did you meet any employees and/or get a feel for the company culture?

2 weeks vacation sounds standard for a new employee, that is after 1 year of service. OP, can you ask them to clarify what the schedule expectations are? Are you permitted to do 8 hr days max and then leave, or are they expecting regular OT and willing to pay for it?
Also clarify the sick policy-is the note needed after just 1 day off? as someone else mentioned, not every illness needs to be looked at by a doctor, sometimes you just feel lousy with a short term ailment and staying home is the best remedy.

Hi All,
I wrote email to the hiring manager about my concerns... he called me back for lunch. It was very nice of him. Any way, here is the new deal, he and HR said, they will give three weeks vacations and increase the salary little more. So it will be 20% now from my current job. He said it's about 40 hour work week but he doesn't care what time I come and go but he wants the projects to be completed. Keep the server running and keep the clients happy. He also said if my kids are sick, I can work from home but I can't do this every week. Once a while is OK.
Regarding sick days, it's three days then he needs some kind of doctors note.
I told him I need 48 hours to make of my mind. But I am starting to like this small company. Please advice, if you were in my shoe, would you take this job?

What if it takes you 60 hours to get the projects completed? Will you get paid overtime? Once you put in your 40 hrs, can you just leave? It sounds like a stressful environment to me.

I want to tell you to go for it......
but I feel like I may be missing something  ??? Is this move for the money? Career path? Closer commute? Quick and dirty it sounds like a "YES", but please tell us WHY you still have concerns  .

How did it go from no sick time and needing a doctor's note to 3 days sick and then the note(which is the norm in most places)?
Are you getting any of this in writing or are you just being told what he thinks you want to hear?
I would ask for that in writing because it contradicts what you posted earlier.
It also sounds like it is going to be more than 40 hours most of the time.
Keeping the clients happy and projects completed sound like more hours.

Sounds to me like this is a salaried position where you will be working 60 hours a week but getting paid for 40 hours a week.

I already asked, they will not pay any over-time. He said 40 hours work week. I have been in the I.T field for over 20 years. When some thing goes wrong or server is dead, I have to work over time to get the server up. It's part of the job. I am OK with this.
The main reason I want to change job is for the salary reason. I need more money. Current company, no matter what I do here, I will not get 20% raise here.
