I don't. No one in my family says "God Bless You" after someone sneezes.
In fact at my last job one of my co-worker's got upset with me because I didn't say "God Bless You" after he sneezed. He was in his own office with his door partially shut. I was sitting in the front office. I could always hear his sneezes because he was one of those annoyingly loud sneezers. But I never said "God Bless You" after one of his loud sneezes.
He asked me why I never said "God Bless You" after he sneezed. I told him I didn't want to and it wasn't required in my job description.
He was taken aback, but never bothered me about the whole "God Bless You" after a sneeze requirement.

I have had that happen, after a loud sneeze but only "bless you." I think in order to be more "politically correct" people are now leaving out the word god to avoid religious implications and make it more of a politeness response to a sneeze.
