For me, one of them wasn't. All they did was cover parking, but I missed the memo that I was supposed to park in the garage. I did street side parking instead. Bad mojo to look at your watch during an interview, but they understood that I was limited to 1 hour before having to go out to refill again, which they did grant me! I had to pay for the drive (no miles reimbursed), food, and hotel stay. It turns out that my future project manager wasn't even aware that I drove 200 miles to interview. She just assumed I was local! In the end, she offered to reimburse me for the meter, but I declined, saying it was only a few dollars. No biggie.
For another, it was thousands of miles away. Their policy was while they do pay for it, I need to pay for it up front. This is protection against paying for the plane tickets, booking the hotels, and then having the interviewee bail on them (which as happened to them before, leaving them to eat the plane ticket costs). They only made exceptions for newly grads who they deemed may be short on cash. They also offered the same courtesy for me, citing that I've been out of work for awhile. I thanked them, but declined, commenting that I had $$ saved up and could foot the bill up front. I presented copies of my receipts, and I was reimbursed in full within a week of returning.
For the latter, they were more desperate than I was to get in employees, so I had that going for me despite being a non-executive/upper management position. Then I actually had a 3rd job out of state, but I got hired without even an in-person interview!!
I've been offered an interview which I would have to fly round trip. Hotel 2 nights and r/t air fare, rental care and meals. Is there a standard who pays for the cost of the trip. Total cost about $650 and change.

I think the key here is the bolded above. There is no standard or norm. Anecdotally, it varies by job, company, location, supply vs. demand, etc.
On the flip side, whether or not you are willing to pay will also depend on one's personal desire for said job.
Some will demand it simply on principal.
You're underestimating big time if you think you can make such a trip for $650! More like $1200--$1500.
How do you figure? I just bought airline tickets for $275 including return flight. Rental car can be $35 a day. Not everyone has to stay at a Marriott. If someone is paying out of pocket, they might be ok with Motel 6.
I feel bad reading the post and replies. I didn't know that many companies are willing to pay for relocation
